The goal of Form & Build is to meet expectations while serving customers with disabilities. Comments on our services regarding how well those expectations are being met are welcomed and appreciated. Feedback can be provided in accessible formats and communications supports, upon request. Feedback may identify areas that require change and encourage continuous service improvements.


Feedback regarding the provision of services to people with disabilities can be made in person, by phone, e-mail, in writing or through the mail. Feedback shall be provided to the Branch Managers and/or General Manager. Management will provide a response no later than seven (7) days of receipt.

Copies of AODA Documents

Accessibility Compliance Reports (ACR) are posted in our branches. Copies of our AODA policies and ACR will be made available upon request. Please contact for a copy.  Form & Build recognizes that people who have a disability use methods other than standard print to access information. Every effort shall be made to provide this policy, or the information contained in the policy, in a format that considers the person’s disability.
